Medicare review
Common questions
Do I need to schedule an appointment before sending my information?
No. Send your updated information first. When your review is ready, we’ll call you to arrange a phone or in-person appointment.
Which form should I use?
Either form works. Option 1 shows the information already in your record so you can review it and make changes. Option 2 is a blank form you fill out yourself. Complete only one.
What if I already sent my updates this fall?
You don’t need to send them again. If something has changed since then, call or email us.
Can my spouse and I share one form?
No. Each person needs a separate form. If you use Option 1, return to this page to start the second person’s form.
Do I have to use an online form?
No. To speak to a licensed insurance agent, call (218) 366-0234 (TTY: 711), or email scott@madseninsurance.com to arrange help with your updates.
What is a Scope of Appointment?
Before an appointment about Medicare Advantage or Part D prescription drug plans, Medicare requires us to document which types of coverage you want to discuss.
You choose the topics. Signing does not enroll you in a plan, change your coverage, or obligate you to enroll.

What happens if I do nothing?
If your plan renews, you generally stay enrolled, but your costs and coverage may change for 2027.
If your Medicare Advantage plan ends and no replacement plan takes effect, you generally return to Original Medicare, without the ending plan’s extra benefits or any drug coverage it included. If a standalone Part D plan ends, you may lose your prescription drug coverage.
If you receive a notice that your plan is ending or that you’re being moved to another plan, contact us so we can review what it means for you.
When can I enroll?
Annual enrollment for Medicare Advantage and Part D prescription drug plans runs October 15 through December 7, 2026, for coverage beginning January 1, 2027.
Other enrollment periods may apply, including if your plan is ending. Contact us to check your deadline.
What should I do about calls and mail from other companies?
Keep notices about changes to your current coverage and any offers you’d like to compare. You don’t have to make a decision during a sales call. If you’re unsure who is calling, hang up and contact the company using a number you trust.
Does the review cost me anything?
No. We don’t charge for reviews or enrollment assistance. We may receive a commission from the insurance company if you enroll through us. Your plan’s premiums and other costs still apply.
